Our Client Forms & Agreement Page is designed to provide easy access to the necessary forms to initiate our work with you.

Clinical counseling clients will complete all paperwork through our HIPPA complaint client portal.

If you are contracting with Kids First for an individualized evaluation of your child, please complete the intake questionnaire below. Additionally, please download and complete both the Consent for Evaluation form and the Health and Education Release form. If you are contracting with Kids First for educational consulting services, please complete the intake questionnaire as well as the Health and Education Release form below.

All PDF forms must be signed and provided to our offices directly or emailed to kim@kidsfirstcollaborative.com. Once we receive the completed forms, we will provide a letter of engagement for your review and signature. Please do not send confidential records until you’ve submitted both the intake paperwork below and your signed letter of engagement.
